■How to Register■

Request "Application Form" to Coordinator and transfer both "Monthly Fee" and "Sports Insurance Fee" to our bank account.
After the due prodeedings, a policy leaflet of sports insurance is forwarded by mail.
Afterwords "Monthly Fee" to be paid to our bank account in the beginning of the current month.
All fees are not refundable.
